feat(database): make the statement span the only span a statement gets - #126
Database opens one client span per statement (0.7.0), but Effect's SQL driver still opened its own span inside it, named after the database and carrying the query text. Every statement produced two client spans, and a consumer that wanted one had to switch the tracer off around its database calls, which also hid the statement span.
The statement span is now the only span a statement gets:
It carries db.query.text (the statement as sent: Postgres keeps $n, ClickHouse inlines values, exactly what the driver's span recorded) and, on failure, db.response.status_code (the SQLSTATE) beside error.type.
The driver call runs with the tracer disabled. fromSqlClient({ driverSpans: true }) keeps the driver's span as a child, for what only it records (server.address, connection waits).
observe runs inside the statement span, so attributes a consumer annotates there land on it. A test pins that, since consumers rely on it to add peer.service and similar.
Plain SQL (Db.sql) spans were named after the database system (postgresql). They are now named after the leading operation and first table the text names (UPDATE items), with db.operation.name and db.collection.name, as the conventions allow for parsed text. The patterns match the ones Maple's warehouse derives query shapes with, so existing shape names carry over.
Minor bump (default trace shape changes). docs/database.md tracing section updated.
Tests
src/database/tracing.test.ts: no driver span by default, db.query.text is the parameterized SQL, SQLSTATE on a syntax error, observe annotations land on the statement span, driverSpans: true nests the driver span under it, and plain SQL is named from its text (with and without a table).
vitest run (1027 passed), typecheck, doc citation/export/example checks.

Summary by CodeRabbit
New Features
SQL statement traces now include query text and clearer operation and table details. Errors can include database response status information.
By default, a statement is represented by a single trace span. SQL driver spans can be enabled when additional detail is needed.
Statement spans can include custom annotations.
Documentation
Updated tracing guidance to explain span naming, attributes, and driver-span options.

Statement span attributes and naming src/database/tracing.ts, src/database/tracing.test.ts, docs/database.md
Statement spans record query text and SQLSTATE response codes. Plain SQL statements derive operation and collection values for span attributes and naming. Tests cover annotations and statements with and without a table.
SQL driver span control src/database/database.ts, src/database/tracing.test.ts, .changeset/statement-span-only.md
FromSqlClientOptions adds driverSpans. Row-returning statements and commands disable driver tracing by default; tests verify that enabling the option nests a driver span inside the statement span.
MapleTechLabs/effect-orm#77: This PR adds statement annotations and span attributes, but does not include the issue’s separate parameter-redaction and full semantic-convention requirements.
MapleTechLabs/effect-orm#8: It introduced fromSqlClient and its options, which this PR extends with driverSpans.
MapleTechLabs/effect-orm#125: It changes Effect SQL span naming, which is related to this PR’s changes to database tracing behavior.

Explanation
The title names the primary change: the statement span becomes the only span a statement gets by default. The changeset covers this. The docs, database.ts (driverSpans option), tracing.ts (db.query.text, plain SQL naming, error attributes), and tracing tests all reflect it. The title is specific and concise, and it fits the 11-word, 70-character length a teammate would scan. It omits the driverSpans opt-in and the plain-SQL naming rule, which is acceptable because it does not need to cover every detail.

statement.sql in the tracing logic, so table extraction and span summaries use
the actual table name rather than text inside a literal.

The statement span now carries db.query.text and, on failure,
db.response.status_code, so the SQL driver's own span (which repeated the
query text under the database name) runs untraced by default. driverSpans:
true keeps it as a child. observe runs inside the statement span, so a
consumer's attributes land there; the tests pin that.
A Db.sql statement has no compiled operation or table, so its span was named
after the database system. It now reads both off the text, as the OTel
conventions allow: the leading word and the first table a from/into/update/
join/table clause names, so `UPDATE items SET ...` is "UPDATE items".

Makes the Database statement span the only span a statement gets: it now carries db.query.text and the SQLSTATE, and plain SQL is named from its text. Turning the driver span off by default also drops the peer attributes the ClickHouse client relied on, and the new table heuristic misreads common SQL.

Findings
🟠 Warning · F1 · Statement spans no longer name the peer now the driver span is off
driver() switches the tracer off, so the span that carried the peer is gone: src/clickhouse-http/client.ts:279 puts db.namespace, server.address, server.port and peer.service on that driver span only. Statement spans keep db.system.name alone, so the service-map edge promised in docs/running-queries.md:110 disappears for every Db caller.
Carry the ClickHouse client's `spanAttributes` onto the statement span — an option on `fromSqlClient`, or an `annotateCurrentSpan` from the client's `execute` — so `peer.service`, `server.address` and `db.namespace` survive.
🟠 Warning · F2 · FIRST_TABLE reads a column or a subquery as the table
FIRST_TABLE takes the first from|into|update|join|table plus a word anywhere, so Db.sqlSELECT extract(epoch from created_at) FROM events`` records db.collection.name: created_at and is named `SELECT created_at`, and `FROM (SELECT id FROM events) t` records `SELECT` — though `docs/database.md:315` says collection is absent over a subquery.
Skip keywords at parenthesis depth above 0 (`extract(epoch from x)`) and a `FROM` followed by `(`, so a column or a subquery never becomes the collection.
